Swimmers MUST have register my athlete forms complete, physical form and $40 fee handed into CHS office before athlete can participate.

Select UTAH, Cedar High, and choose Boys or Girls Swimming. Follow directions to E-sign waivers, print off physical form to take to doctors appointment, and pay $40 on line or directly to CHS office.
Those who are listed as incomplete my need to double check that you have selected sport (Swimming)
If it says your fee is missing and you have paid - do not worry it may just be the school is slow recording. If you have turned in your physical form and it is not showing do not panic - the school has to manually add those into the system.
