Dr. Bruce Hendrix will be available for physical exams on Wednesday OCTOBER 31 @ 3:00 P.M.

YOU MUST HAVE YOUR FORM FILLED OUT AND SIGNED BY PARENTS BEFORE DR. HENDRIX CAN DO EXAM.

GO TO EARLIER POST AND THE LINK ON UTAH HIGH SCHOOL ATHLETIC  ASSOCIATION WILL TAKE YOU TO THE FORMS, AND YOU CAN PRINT.  FIRST TIME ATHLETES FILL OUT FORM A, AND RETURNING ATHLETES FORM B. 

Health Forms

Swimmer  need to have a health form completed and handed in before you can participate in the first swim meet - November 9.    If you are a first time CHS swimmer and have not completed a health form for any other high school sport you need to hand in Form A if you are a returning athlete you need Form B.
PLEASE THINK AHEAD AND GET A DOCTORS APPOINTMENT SCHEDULED NOW.

 Both of these forms are available to print and take to your family physician at this link http://www.uhsaa.org  On the Utah High School Athletic Association web site you will click on "Sports Medicine" in the top header.   Then scroll down to the Medical Forms and you will find Form A & B. Form A must be filled out by first time athletes, and Form B is filled out if there is an A Form on file from a previous year.

Those forms need to be handed into Coach Coston or CHS front desk, ASAP.
